Sample Forms for Rescuers

These templates are provided as models which you may adapt to suit your situation. Some of the questions may apply to all animals, while others are important for some but irrelevant for others, so use what you need and modify or discard the rest. These documents are also available for free download on my web site at www.sheilaboneham.com/rescueforms.html. I would also recommend that you look at the forms used by other organizations that work with the same type of animals you do – many have learned from experience and have added or changed the questions they ask accordingly.

Make your forms complete enough to provide the information you need. After all, if a would-be adopter or volunteer can’t spend a little time to fill out the application, how much time will he or she spend when things don’t go smoothly with an animal? I also tend to prefer open-ended questions to multiple choice – you often learn a lot more about people when they use their own words.

I have included a few comments and suggestions within the forms in boldface. Substitute you specific information, or delete as you please. And if you have suggestions to improve these templates as they appear on my web site, please let me know – sheilaboneham@comcast.net.
